The Opportunity:
We are looking for Senior Staff Nurse (Cardiology Specialty Nurse) to join us to provide high quality patient care.

Job Responsibilities:
The role of the incumbent is to assess the eligibility of all cardiac patients and successfully recruiting of patients into the Cardiac Rehabilitation/Heart Failure Programme, offer inpatient education and subsequent follow up in clinics. The incumbent is accountable to the Cardiology Services Director and the Nursing Department

A. Cardiac Rehabilitation (CR)

Provide CR Phase I counselling to all post angioplasty patients.
Ensure all necessary follow ups are ordered/ given.
Assess and review patient’s medical history and medications.
Develop comprehensive individualized Rehabilitation plan for patients and ensure patient agrees to the programme in tandem with other stakeholders.
Co-ordinate rehabilitation appointments and follow through the patients in the programme and ensure compliance to 8-week plan in coordination with PT/OT.
Respond to concerns and queries regarding treatment plan for the Cardiac Rehabilitation Programme. Explore and allay anxiety to return to normal life style and work.
Reinforce compliance to diet lifestyles changes, medications

B. Heart Failure (HF) Programme

Provide daily screening of Cardiologist list to identify suitable patients for HF Education
Assist to screen all eligible patients suitable for Intravenous Ferritin replacement and all patients due for yearly Influence and Pneumococcal vaccinations
Assist to make appropriate recommendations prior to discharge and ensure outpatient follow up are planned.
Conduct HF Post Discharge Telephone Call with 1 week, post discharge.
Assist in the Cardiologist –Led HF Clinic once a weekly.

C. Rapid Access Chest Pain Clinic

Provide a One-Stop Chest Pain Service for Polyclinic referrals.
Review patients independently in the RACPC clinic
Ability to perform treadmill independently
Close collaboration with NUP colleagues and Clinical Operations in patient suitability and appointment scheduling.

D.Post Myocardial Infarction (STEMI/NSTEMI) Clinic

Assist to assess punctured site for post complications and manage modifiable CVRFs (HbA1c, BP, Lipid profile within targets).
Assist to assess side effects from Dual Anti-Platelet Therapy.

E. Expansion of new services

To work with Clinical /Service Operations on the processes to set up new clinic and expansion of existing clinics.

F. Clinic roster planning for Nurse-Led Cardiology Services.

Ensure adequate coverage to avoid compromising the services.

Job Requirements:
Min Diploma in Nursing or State Registered Nurse Certificate
Relevant Advanced Nursing Diploma or equivalent is preferred for candidate applying for the Senior Staff Nurse position
