Responsible for performing care assessment, planning and management in accordance with established ambulatory nursing policies, standards and evidence-based practices. Collaborates with the interdisciplinary team and community partners to achieve quality holistic care. Facilitates care transition of patients and coordinates with various stakeholders and community resources. Provides patient, family and/or caregiver education as well as promotes health and well-being.
Responsibilities and Activities
1 Person-centred Care
- Perform biopsychosocial and environmental assessments to identify strengths and needs of patients, families and/or caregivers
- Communicate with the patient’s healthcare providers, families and/or caregivers to understand patients’ needs
- Develop person-centred nursing care plans with consideration of patients’ needs and preferences
- Communicate care plans and address concerns from patients, families and/or caregivers
- Perform evidence-based nursing interventions as prescribed in the care plan
- Perform medication administration, titration in accordance with organisational guidelines and protocols
- Support patients’ ability for medication self-management and medication adherence and escalate to relevant care team members for medication reconciliation and optimisation
- Manage and escalate appropriately in the event of unexpected and/or abnormal changes
- Maintain timely and accurate documentation
2 Patient Engagement and Education
- Involve patients, families, caregivers and/or relevant others as active partners, to identify needs, preferences and expectations of the patients
- Build therapeutic relationships and maintain professional boundaries when dealing with patients, families and caregivers
- Provide effective training and education to patients, families and/or caregivers according to self-management needs
- Identify patients, families and/or caregivers who are suitable to adopt technologies, and support relevant training
- Engage relevant stakeholders to educate and/or empower patients, families and/or caregivers for self-management
3 Nursing Practice Management and Operational Excellence
- Assess his/her practices against established organisational guidelines and standards to improve self-efficiency
- Identify risks/hazards and take appropriate measures as per established policies and procedures to ensure safety at the workplace
- Practise safety and infection control precautionary measures
- Support early identification of health threat risks to patients and/or population through information and/or data gathered
- Identify and report risks and barriers to the safety of patients in accordance with organisational protocols
- Execute responsibilities as per emergency protocols in the event of public health threat or emergency
- Comply with guidelines and policies to ensure patient confidentiality and personal data protection

- Underpinning Knowledge
- Demonstrate knowledge and understanding of anatomy and physiology of normal skin and its appendages
- Dermatological Assessment and Investigation
- Have an awareness of how to assess a patient with dermatological condition
- Have an awareness of common dermatological investigations for diagnostic purposes such as: Bloods for haematology and biochemistry; Swabs for microbiology and virology; Skin tissue for pathology; Samples for mycology, eg, skin scrapings and nail clippings.
- Caring for the patient with a dermatological condition
- Recognise and describe signs and symptoms of common dermatological conditions
- Have an awareness of factors influencing a professional relationship with patients and carers
- Have an awareness of clinically safe practices when caring for an ill patient

Job Requirements
Education and Training: Diploma/Degree in Nursing
Professional License: State Registered Nurse with Singapore Nursing Board
Preferably prior derma experience / derma trained
